Respiratory Services Transition Process

PromptCare Respiratory can accept a referral in many ways:

  • Call (877) 776-6782 for referrals going home to Northern New Jersey, New York, Connecticut, or New England and speak with a dedicated patient admissions representative.
  • Call (888) 278-7702 for referrals going home to Southern New Jersey, Pennsylvania, Ohio, West Virginia, Delaware, or Maryland and speak with a dedicated patient admissions representative.
  • Fax (800) 889-0862 for referrals going home to Northern New Jersey, New York, Connecticut, or New England.
  • Fax (877) 782-8263 for referrals going home to Southern New Jersey, Pennsylvania, Ohio, West Virginia, Delaware, or Maryland.

Our high-tech customer care managers will take it from there. They will:

  1. Verify insurance benefits.
  2. Contact the family to introduce PromptCare and schedule a home safety assessment.
  3. Arrange for essential equipment to be delivered to the hospital for training and/or transportation purposes.
  4. Schedule all needed training with the family.
  5. Obtain all necessary documentation to satisfy insurance and regulatory requirements.
  6. Obtain all required authorizations from the insurance company.
  7. Arrange for ordered supplies and equipment to be delivered to the home prior to discharge.
  8. Schedule a respiratory therapist to meet the family at home on the date of discharge.

From start to finish, PromptCare takes care of all the critical transition steps, so you don't have to.
